I plugged it into the USB with a wire to its tail to charge it. Ready to go in about half an hour, I left the factory settings intact for sensitivity and speed.
I plugged in the 1/4 in stub usb receiver, removed the cord and it moved the cursor immediately no setup or messing around [Dell laptop]. Now I got a surprise.
Unlike my other wireless trackball, this acts as a regular wireless mouse too. Move it around the desktop and it works great. Keep it stationary and move the trackball and the mouse moves well in this mode too! It also has a scroll wheel which is very helpful and is rotated by the thumb [actually by the joint closest the hand just beyond the first metacarpal.]
Then, I picked it up and used it as a handheld and it was very comfortable. It worked surprisingly well this way but I don't do much gaming and need both hands to type so it sits on the desk.
Now something this weird takes a bit of getting used to. At first I was moving the trackball and the mouse at the same time. It follows both directions but I can't think fast enough to provide two simultaneous sets of directions. I learned to do one or the other and have settled on moving the mouse instead of the trackball in most cases. When the laptop goes to the TV chair, this device is perfect. The USB stub is so small it does not get hung up like a thumb drive may. Also, I go into trackball mode with it sitting on the upholstered arm of my chair.
The one issue I have is that my hands are a little larger than average and the unit is designed for a smaller hand. If I turn my hand up when holding the device with fingers on triggers and thumb on trackball, the device lays across my fingers not touching my palm. The longer I use it, the more comfortable it gets. I realize that I do not have to manhandle it like I do my Logitech trackball. I am sure that women will be much better with this size wise and coordination wise. It is more like a sketch charcoal than a hammer light fingered folks will love it. If you need to grab and grip, this mouse may not be for you.
I have been using it for a while and just realized why I really like this little fellow no batteries required. It has an off/on switch but who really pays attention to that. I leave mine on and will plug it into the usb for 45 min and it's good for another stretch. I hate buying batteries!I bought this seeking relief from ever-increasing pain in both hands. (My right hand had begun to ache so much I started alternating with my left.) I plugged this in and after making a simple adjustment to the trackball speed and scroll wheel operation everything was fine. I don't know what 9-out-of-10-Doctors would recommend, but I have not any pain in either hand since I started using this mouse. It's easy to use off-surface with either hand and can also be used more conventionally and placed flat on a surface.

This may have been unique to me because of the seller that fulfilled the order (BlueMall), but mine arrived in a shoddy unlabeled box basically shoved inside a large envelope and stuffed inside of my mailbox. It had been crushed pretty thoroughly by the time it arrived and they didn't even bother labeling it 'do not crush' on the package. The actual device did not function, as over about 2 feet from the device it would disconnect constantly, which meant I couldn't actually pick it up from my desk to use it in the fashion that it is meant to be used. Of course, many others are saying the device works well, so mine might have been damaged due to improper packaging. I attempted to use it with the USB plugged into my computer, but apparently it will not work while it is plugged in to charge.When I contacted the seller, they wanted me to call a phone number that I couldn't connect to instead of helping me with email. They didn't offer real assistance until I left negative seller feedback, at which point they gave me a return label to print out so I could return it. Notably that was only offered after I wasn't satisfied with a partial refund. I feel like they would not have assisted me at all had I not left negative feedback, only wanting to convince me to remove it.
Even with packaging and seller issues aside, I did not find this device to be usable as a regular trackball/mouse, it is not comfortable to hold if it is on the desk, so I would not recommend getting it unless you intend to always use it standing. This device just attempts to do too much and ends up only being half-way decent at each thing rather than really good at one.

I had a VERY difficult time using this mouse, it doesn't feel right in your hand. But I have to give the company credit, they hav EXCELLENT customer service and have tried really hard to help me use their product
